Referral letter from your GP
In order to have your invoice reimbursed by your health insurance company, a valid referral from your General Practitioner is necessary at the time of your registration.
This must include the type, name, address, signature (plus stamp of the practice) and AGB-code of the referrer, as well as your name, address, gender, date of birth and policy number / health insurer (uzovi number). It must mention if there is a suspected DSM-appointed disorder present, and it has to be a referral to either general basic mental health care (gb-GGz) or specialised mental health care (g-GGz).
The referral letter must contain a reference to our practice and the date on the referral must not be more than 9 months before the time of signing up. It’s therefore usual that you already have this referral when you contact the practice. Only then can an eventual intake procedure take place.
