Referral letter from your GP
In order to have your invoice reimbursed by your health insurance company, a valid referral from your General Practitioner is necessary.
This must include the type, name, address, signature (plus stamp of the practice) and AGB-code of the referrer as well as your name, address, gender, date of birth and policy number / health insurer (uzovi number). You must mention if there is a suspicion of suffering from a DSM-5-appointed disorder, and it has to be referred to as general basic mental health care (GBGGZ) or specialist mental health care (SGGZ).
Then it is important that a date is stated on the referral letter and that this date is no more than 6 months before you get in touch with us, before the start of the treatment (intake procedure). It is therefore common for you to have this referral when you contact our practice.
