Referral letter from your GP
In order to have your invoice reimbursed by your health insurance company, a valid referral from your General Practitioner is necessary.
This must include the type, name, address, signature (plus stamp of the practice) and AGB-code of the referrer as well as your name, address, gender, date of birth and policy number / health insurer (uzovi number). You must mention if there is a suspicion of suffering from a DSM-appointed disorder, and it has to be referred to as general basic mental health care (GBGGZ) or specialist mental health care (SGGZ).
The referral letter must contain a reference to our practice and the date must not be more than 6 months before the time of signing up. It’s therefore usual that you already have this referral when you contact the practice.
